University of Nevada Las Vegas Athletic Training

Dear Parents/Guardians and Student-Athletes:

On behalf of the Intercollegiate Athletic Department at the University of Nevada Las Vegas, the athletic training staff would like to welcome you and hope you enjoy an injury free career. The following information is EXTREMELY IMPORTANT! We need your time and cooperation to carefully and accurately complete all the requested information and forms. Please download the appropriate forms and return them to the athletic training department. (Links for forms below).

The UNLV Department of Athletics provides secondary medical insurance coverage for all student-athletes, provided the injury and/or illness is athletically related. The following information is designed to help explain the insurance coverage, referral procedures and any other questions that you may have concerning our athletic injury coverage.

Requirements:

  • All non-athletic scholarship student-athletes are required to have primary insurance that covers athletically-related injuries.
  • A clear photo-copy of the front and back of their insurance card and must be done yearly.
  • Please note that the insurance that can be purchased through the UNLV Student Health Center does not provide coverage for injuries or illnesses sustained during intercollegiate athletics. This plan may not be considered as primary insurance.
  • Annual Insurance Information must be filled out and signed by the parents/guardian of the student-athlete and the student-athlete.
  • Please also note that the NCAA's Catastrophic Injury Insurance Program covers student-athletes who are catastrophically injured while participating in a covered intercollegiate athletic activity (subject to all policy terms and conditions). The policy has a $90,000 deductible and is supplemental coverage in the event of a catastrophic injury. More information on this program can be found on the NCAA's web-site at www.ncaa.org

Insurance coverage carried by UNLV Athletic Department:
The Athletic Department, as mentioned above, provides secondary medical insurance coverage for all student-athletes. The policy provides secondary coverage on injuries and/or illnesses that are the direct result from intercollegiate practice or competition.

This policy will be applied only to medical expenses for services rendered by a participant in our network and to those providers deemed necessary. A claim must be submitted to your primary insurance. For those Athletic scholarship student athletes that don't have primary coverage, the coverage provided by UNLV will become the primary insurance. The Athletic Training Staff must approve all referrals to medical personnel. Failure to gain advance approval will result in no-payment of incurred charges. Medical expenses for injury and/or illness not a result of athletic related activities that occur are not covered nor are they the responsibility of UNLV.

Filing a Claim:
The UNLV insurance policy requires an initial claim be filed with the student athlete's primary insurance. After review of the medical claim, it is customary for the student-athlete's primary insurance to process a payment. During this processing period, it is important to forward all medical bills received to UNLV. When the student-athletes primary insurance has made a payment for the claim, UNLV will be responsible for the remaining balance.

THIS INFORMATION MUST BE COMPLETED AND RETURNED BEFORE YOU WILL BE PERMITTED TO PARTICIPATE IN PRACTICE WITH YOUR RESPECTIVE TEAM.
